Guidelines for a Screening Colonoscopy Referral:

Patients are eligible if:

  • They have completed a fecal occult blood test (FOBT) kit and the results are positive and they are between 50-74 years of age.
  • They have a first-degree relative (parent, sibling or child) who had colorectal cancer. A patient’s first colonoscopy should be at age 50 or 10 years prior to their family member’s age at diagnosis.


1.  Physician to complete referral.  Screening Colonoscopy Referral Form-March 2018 

2.  Fax referral to the Diagnostic Assessment Program at 807-684-5810. The patient will be contacted directly by a qualified health care professional to organize the colonoscopy booking.

3.  Completed referral forms will be filed on the patient’s health record. 


Looking to refer a patient for a ERCP?

Currently Dr. Harris is the only surgeon in Thunder Bay who preforms the ERCP procedure.

If you are a health care provider looking to refer a patient for a urgent ERCP please contact TBRHSC switchboard at 684-6000 and ask to speak with Dr. Harris directly. All other non-urgent referrals may be sent directly to his office by fax to 807-346-1445.
